RADIAS Health is hiring a full-time Waivered Services Case Manager to join the Waivered Services Case Management team. The role involves assisting clients on waivers to access necessary services, supporting clients at risk of placement in facilities, and coordinating services to enable clients to live in the least restrictive settings. The position offers a hybrid work schedule and a starting salary range of $51,517 - $55,036 plus benefits.

Responsibilities include:
  • Helping clients access waiver and state plan services and other supports
  • Developing and implementing care plans based on assessments and client needs
  • Informing and assisting clients and guardians in service options and provider access
  • Coordinating and arranging services and documenting all activities
  • Providing crisis intervention and responding to Vulnerable Adult reports
  • Monitoring client progress and adjusting services as needed
  • Maintaining accurate case records and data tracking
Requirements:
  • Bachelor's degree in behavioral sciences or related field, or
  • Bachelor's degree with one year of relevant experience
  • Valid driver's license, car, and auto insurance
