Job summary

New River Community Services is seeking a Transitional Care Case Manager to provide case management and clinical services for individuals with mental illness, substance use disorders, or developmental disabilities and their families.

The primary function is to ensure engagement in and transition to clinic-based care for consumers discharging from psychiatric inpatient care or crisis services, including mobile crisis teams.

Qualifications

• High school diploma required with at least one year of experience in a setting working with individuals with substance use disorders and/or possible co-occurring mental illness.
• Bachelor’s degree in education, social work, psychology or human services related field is preferred.
• Registration as a QMHP or trainee is preferred.
• Additional languages may be a plus.

Responsibilities

• Provide case management and clinical services for individuals with mental illness, substance use disorders and/or developmental disabilities and their families.
• Ensure engagement in and transition to clinic-based care for consumers discharging from psychiatric inpatient care or crisis services, including mobile crisis teams.
• Office and home visits, referrals and connection to needed services, crisis support and response, assessment and care coordination.
• Develop individualized service plans and provide individual and family support and psychoeducation.
• Coordinate transportation and services, with may continue up to 60 days post discharge.

Starting Salary Range: $19.93 - $22.92 hourly-depending on education and experience.
