CAMBA in Brooklyn, NY is seeking a Case Manager to provide comprehensive case management services to clients. The role includes intake, assessments, service planning, referrals and advocacy to connect clients with housing, health, and legal resources.

Stability Supportive Housing Programprovides 60 units of scattered site supportivehousingto chronically homeless single adults and adult families withafamilymemberwho has a serious mental illnessand/or substance use disorder. Theapartments are locatedthroughout New York City.



Position: Case Manager

Reports To: AssistantProgram Manage

Location: Brooklyn NY 11225

WhatDoesaCase ManagerDo:


  • Provide all contract-funded activities, including conducting Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conferences, and other program requirements.

  • Complete all program documentation and enter client services into Client Track, ensuring records are accurate and up to date.

  • Ensure clients are connected to and maintain primary medicalcare, andadhere to medical and medication treatment plans.

  • Connect clients to needed services, suchas,medical, mental health, substance use treatment, as wellas,legal, entitlements, etc.

  • Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes.

  • Function as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.

  • Escort clients to appointments (educational, medical, social service, etc.)

  • Assist clients in completing applications for benefits and entitlementsand monitors outcome of process.

  • Collect all required documentation and create client files.

  • Conduct aminimumone home visit per month

  • Conduct program required unit inspections as required by funding source.

  • Request Emergency Assistance to assist clients in paying clients’ portion of utility bills, if needed.

  • Input work orders into the system and follow-up on repairs, which may include contacting building management and coordinating between outside contractors and clients

  • Monitor clients’ living conditions.

  • Respond immediately to emergencies and inform supervisor.

  • Meet required monthly projections.

  • Provide all required information for the completion of monthly program reports.

  • Maintain clients’ charts always audit ready.

  • Provide all required information for weekly/monthly/quarterly/annual reports to CAMBA management and/or to funders.

  • Participate in administrative and staff meetings as requested.

  • Other duties as assigned.



What are the Qualifications for the Position:


  • Bachelor’s degree with one year of experience, or

  • Associate degree with two years of experience, or

  • High school diploma or GED with four years of experience



Minimum Experiences and Skills Required:


  • Experience as described above, preferably in a mid-sized (250+ employees) organizationworkingwith persons with ahistoriesof homelessness, mental illness, and/or substance abuse.

  • Goodwritten & verbal communication.

  • Computer literacy in Microsoft Office Suite.



Preferred Qualifications:


  • Bi-lingual English and Spanish.



Compensation: $54,672.80 annually


Status: Full-time (35 hours weekly)
