Job summary

The Center for Inherited Blood Disorders (CIBD) is seeking a Care Manager Supervisor to lead the case management team and ensure the delivery of high-quality patient care. You will supervise nursing staff, coordinate daily operations, and help shape clinic policies to meet Joint Commission and other regulatory standards.

Qualifications

  • Bachelor’s degree from an accredited school in Nursing or equivalent.
  • Minimum five years of experience in healthcare as a Registered Nurse, including at least three years in an ambulatory setting.
  • Minimum two years experience as a lead or charge Registered Nurse.
  • Minimum two years experience as a Case Manager or equivalent position.
  • Current Registered Nurse License issued by the State of California.
  • Basic Life Support (BLS) Certification.
  • HIPAA Training (or ability to complete within one month of hire).
  • Preference for familiarity with Title 22, Joint Commission, and related regulatory standards.

Job Summary

Reporting to the Clinic Manager, the Care Manager Supervisor ensures the delivery of high‑quality patient care by supervising the case management team and managing the daily, comprehensive care management operations and services. As an administrator, the Care Manager Supervisor will assist in the development and implementation of policies and procedures related to patient care that meet local, State, Federal requirements and other accreditation standards (e.g. Joint Commission, National Patient Safety Goals, ISMP, Title 22, CCS/GHPP/Medi‑Cal). The Care Manager Supervisor will work with the Clinic Supervisor and Nurse Educator to lead the development and implementation of educational materials and in‑services for staff and patients, and participate in Quality Improvement projects and activities. The Care Manager Supervisor works collaboratively with the clinical teams to support the physical and psychosocial well‑being of patients and their families.

Essential Duties
  • Supervision: Directly supervises case management team (including but not limited to Nurse Care Managers and Patient Care Coordinators) through the use of clinical, management, and leadership skills. Supervises the daily, case management operations and services. Delegates Nurse Care Manager assignments of new patients, or upon patient request, and coordinates daily triage assignments. Conducts hiring, scheduling, evaluating, training, and developing the case management team.
  • Care Management: Promotes CIBD's comprehensive care model by overseeing the care management and ensuring they are trained in government and other regulatory agency requirements and cross‑trained in job skills to ensure adequate care management coverage. Ensures patients are receiving the highest quality hematology care to promote longer, healthier and more productive lives. Provides coverage for case management assignments when required by staffing. Reviews and redirects incoming labs, imaging and other patient related services and ensures the providers and Care Managers follow up on these services.
  • Administrative: Works collaboratively with the physicians, case management staff, clinical staff and CIBD leadership. Participates in the development and implementation of Clinic workflows, policies, procedures, and guidelines for care management staff, training and education for patients and their families, and Joint Commission and other regulatory agency readiness.
Requirements
  • Bachelor’s degree from an accredited school in Nursing or equivalent degree or equivalent years of experience in nursing.
  • Minimum five years of experience in healthcare as a Registered Nurse, including at least three years of experience in an ambulatory setting.
  • Minimum two years experience as a lead or charge Registered Nurse.
  • Minimum two years experience as a Case Manager or equivalent position.
Required Licenses/Certifications
  • Current Registered Nurse License issued by the State of California.
  • Basic Life Support (BLS) Certification.
  • HIPAA Training (or ability to complete within one month of hire).

  • Excellent verbal and written communication skills to communicate effectively with patients and healthcare associates, outside funding agencies (Federal, state, local, industry and community), vendors, staff at all levels and the general public.
  • Strong organizational skills in the areas of administration.
  • Ability to interpret and apply federal regulations and organizational policies and procedures relative to the administration of sponsored projects activities.
  • Ability to work independently with excellent interpersonal and team building skills and effective staff training, leading and mentoring skills; and proven ability to formulate and implement policies.

Compensation listed is the expected salary base at time of hire. Actual compensation offered will be determined by factors such as location, level, job‑related knowledge, skills, and experience.

The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD), a 501(c)3 non‑profit organization, is committed to providing comprehensive care for patients and their families with rare, chronic inherited blood disorders. CIBD is a part of a nationwide network of over 140 Hemophilia Treatment Centers that use multidisciplinary teams of blood disorder specialists to provide expert comprehensive diagnosis, medical care, outreach, education, outcomes monitoring, surveillance, and prevention services to people with Hemophilia, Sickle‑Cell Anemia, Thalassemia, Von Willebrand Disease, Mast Cell Activation Syndrome (MCAS) and other related benign hematology conditions. CIBD is the regional grantee for the Western States Region for Hemophilia, the Pacific Sickle Cell Regional Collaborative (PSCRC) and Networking California for Sickle Cell Care (NCSCC) providing regional leadership, fiscal and administrative oversight, grants management, capacity building, clinical education, technical assistance, evaluation, and outreach to public and private health agencies at the local, state, and federal levels.
