Job summary

The Case Manager will work within the Montgomery County Supervision Services to engage individuals under supervision and/or monitoring with Pretrial Services, Adult Probation and Parole, and/or Treatment Courts who may benefit from access to substance use and mental health services.

Qualifications

  • Bachelor degree in Human Services, Social Work or related field required.
  • ASAM Level of Care Certification preferred/required.
  • Strong communication and organizational skills are essential.
  • Experience with behavioral health and/or IDD services is preferred.

Responsibilities

  • Conduct evidence-based substance use and mental health assessments to determine care needs.

Job description

Summary

The Case Manager will work within the Montgomery County Supervision Services to engage individuals under supervision and/or monitoring with Pretrial Services, Adult Probation and Parole, and/or Treatment Courts who may benefit from access to substance use and mental health services.

The Case Manager will assess the individualized clinical needs of the defendant by conducting evidence based assessments (substance use and mental health) to determine level of care needs, any service gaps, and recommend strategies to ensure proper implementation of individualized treatment plan. The Case Manager will facilitate all required protocols to ensure appropriate connection to treatment including; securing funding and identifying placement to the appropriate programs and resources.

Essential Duties And Responsibilities
  • Conduct evidence based substance use and mental health assessments to determine level of care needs and assist in the development of individualized treatment plans.
  • Refer to and facilitate the appropriate level of care in order to obtain necessary treatment.
  • Complete pre-authorizations with insurance providers and/or secure funding for all necessary treatment.
  • Complete documentation as required by Department of Drug and Alcohol Programs-Commonwealth of Pennsylvania, and any funding and/or licensing/ accreditation entities.
  • Refer and facilitate connection to community recovery resources (communities of recovery, educational, vocational, social, cultural, resources, mutual self- help groups, professional services, etc.).
  • Respond to emergency and/or crisis situations and facilitate appropriate interventions; connecting individuals with resources and navigating the systems.
  • Assist with reintegration into a prosocial life in the community.
  • Maintain cultural competency and provide trauma-informed care.
  • Uphold confidentiality regarding information received during the facilitation of treatment services – maintain current understanding of federal, state and local confidentiality rules and regulations, and maintain signed release of information forms.
  • Participate as a member of a multidisciplinary team designed to provide services from a holistic and Values based perspective.
  • Maintain project logs, reports and accurate records in appropriate files and database(s), adhering to Department of Drug and Alcohol Programs-Commonwealth of Pennsylvania standards.
  • Collaborate with agencies and institutions to provide effective treatment support services to identified individuals.
  • Develop and maintain a rapport with a variety of county and state agencies (i.e. County Health and Human Services Departments, Montgomery County Correctional Facility, Access Services), as well as treatment service providers.
  • Interact through written and verbal correspondence with the Judiciary and other stakeholders.
  • Complete 40 hours of professional development training annually as requested by state standards.
  • Undergo and maintain successful: Federal Bureau of Investigation (FBI) Criminal History Clearance, PA Child Abuse History Clearance, and MCCF clearances when applicable.
  • Complete training and maintain compliance with Mandated Reporting Regulations through PA Child Protective Services Law.
  • Perform other duties, tasks and special projects as required.
Qualification Requirements

  • Bachelor Degree in Human Services, Social Work or other related field required; or an equivalent combination of education and experience may be substituted.
  • One year Case Management experience preferred.
  • Two years' general work experience providing support to individuals in behavioral health and/or IDD services preferred.
  • Obtain and maintain an American Society for Addiction Medicine (ASAM) Level of Care Certification.
  • De-escalation and Crisis Intervention Skills.
  • Undergo thorough pre-employment background investigation including satisfactory physical examination and drug screen.
  • Knowledge of department policy and procedures.
  • Excellent oral and written communication skills.
  • Strong Organizational skills.
  • Knowledge of mental health and drug and alcohol systems and evidence based treatment interventions.
  • Knowledge of treatment programs and government agencies.
  • Knowledge of the criminal justice system.
