Firetree, Ltd. in Beavertown, PA is seeking a case manager to support clients in the inpatient substance use disorder program. This full-time, non-exempt role coordinates appointments, assists with obtaining IDs, and helps plan aftercare and housing options.

The position requires a high school diploma, strong computer skills (Word, Excel, Outlook), and the ability to work with clinical staff. You will participate in group sessions, coordinate family calls, and assist with UR processes to ensure

  • High school diploma or equivalent, experience in a health or human service agency, preferably in a drug and alcohol setting preferred.
  • Strong computer skills (MS Word, Excel, and Outlook) and ability to learn other software as required.

Responsibilities

  • Coordinates mental health and medical appointments.
  • Assists clients in securing personal identification such as driver’s license, social security card and other government IDs.
  • Coordinates any legal components, costs, fines, etc. necessary for clients.
  • Conducts multiple group sessions (1-2 per week) as needed.
  • Serves as coordinator for family calls/meetings.
  • Participates in clinical consultations.
  • Locates and coordinates potential housing options for clients.
  • Serves as UR backup as needed.
  • Completes seven day aftercare calls with discharged clients.
  • Initiates and coordinates, with UR staff aftercare planning for clients.
  • Performs other duties as assigned.

Summary

The primary purpose of this position is to provide case management services to clients in the substance use disorder inpatient program. All activities must support the Firetree, Ltd. mission, strategic goals, and objectives.

Essential Duties and Responsibilities

The position duties and responsibilities listed below describe the general nature and scope of work. Other responsibilities, duties and skills may be required and assigned, as needed.

  • Coordinates mental health and medical appointments.
  • Assists clients in securing personal identification such as driver’s license, social security card and other government IDs.

Education and/or Experience

High school diploma or equivalent, experience in a health or human service agency, preferably in a drug and alcohol setting preferred. Individual will be assigned additional training and may be required to seek additional certifications.

Technical Skills

To perform this job successfully, the employee should have strong computer skills (MS Word, Excel, and Outlook). Must have the ability to learn other computer software programs as required by assigned tasks.
