Telecare Corporation is seeking a dedicated Case Manager Substance Use Counselor Certified to support individuals with mental illness and substance use challenges. You will help clients live more independently in their community, coordinating care and linking to ongoing services.

The role requires CADC or SUDCC certification, CPR/First Aid/CPI, and a valid driver’s license. Shifts are full-time with AM hours in California, with competitive starting wages and geographic pay differentials.

Qualifications

• High School Diploma or GED is required.
• Certification as a CADC I, II, or III or SUDCC I, II, III or IV approved by DHCS and NCAA is required.
• Authorized to bill for drug medical programs and learn necessary billing codes within 2 months.

Responsabilités

• Serve as primary support and case manager for individuals with mental illness with focus on substance abuse.
• Assist clients to live as independently as possible in their chosen community.
• Engage clients to connect with ongoing substance and/or mental health services and ensure warm hand-offs to other community services.
• Provide documentation and follow-up as part of discharge planning and continuity of care.

What You Will Do to Change Lives

The Case Manager Substance Use Counselor Certified serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community, focusing on substance abuse and addictions

Shifts Available:

Full-Time | AM | Shifts: 7:00 AM - 3:30 PM | Days: Monday - Friday

Expected starting wage range is $27.85 - $34.42. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Diploma or G.E.D and one (1) year experience in a treatment environment for mental health and drug and alcohol issues
• Certification as a CADC I, II, or III or SUDCC I, II, III or IV approved by Department of Health Care Services (DHCS) and the National Commission for Certifying Agencies (NCAA)
• Authorized to bill for drug medical programs, and within 2 months of employment learn necessary billing codes
• All certifications must be maintained for duration of employment
• Within two (2) months of employment, ability to apply financial requirements for billing and learn billing codes
• Sensitivity to multi-cultural populations and issues
• Must be credentialed as a qualified mental health professional, where applicable
• Must be at least 18 years of age
• Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care and maintain current certification throughout employment
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Clinicians and peer staff will engage clients to connect with ongoing substance and/or mental health services, ensure linkage and \"warm hand-offs\" to other community-based services, and post-discharge follow-up.
