Overview

We are seeking a dedicated and compassionate Case Manager to join our Special Child Health Services team. This role is essential in supporting Children and Youth with Special Health Care needs and their families by coordinating services that promote optimal development, health, and well-being. The ideal candidate will be a licensed nurse or social worker with experience in pediatric case management and fluency in Spanish. You will work closely with families, service providers, and community resources to develop, implement, and monitor individualized care plans.

Responsibilities:

• Needs Assessment: Collaborate with parents and Children and Youth with Special Health Care needsto assess service needs across developmental, educational, medical, and emotional domains.

• Individual Service Planning (ISP): Create and update individualized service plans tailored to each child’s unique needs and family goals.

• Service Coordination: Actively assist families in accessing services by making appropriate referrals and helping navigate available resources.

• Crisis Support: Serve as a responsive resource during family crises and help resolve complaints by providing information and service alternatives.

• Collaboration: Foster effective communication among the child's care team, including primary care physicians, specialists, educators, and therapists.

• Monitoring Progress: Conduct regular reviews to evaluate the child’s progress toward goals and adjust the ISP as needed.

• Community Engagement: Build and maintain partnerships with local and regional organizations that serve Children and Youth with Special Health Care needs.

Qualifications:

• Licensure:

  • Valid New Jersey RN license OR

  • Valid New Jersey CSW, LSW, or LCSW license.

• Education:

  • Bachelor’s or Master’s degree in Nursing (BSN/MSN) OR

  • Bachelor’s or Master’s degree in Social Work (BSW/MSW).

• Experience:

  • Minimum 1 year of pediatric case management experience with Children and Youth with Special Health Care Needs.

• Language Requirement:

  • Bilingual in Spanish preferred.

The anticipated starting base pay for this position is:

• $55,000-61,000 per year, depending on experience and qualifications.
