Overview

The Social Worker is assigned to specific clinical care units and/or specific patient populations to provide services to patients and families whose medical, psychological, and/or social situation impacts compliance with the physician care plan, the provision of appropriate health care services for the patient, and/or timely discharge. Coordinates the assistance necessary to meet patient-specific needs related to placement, basic living resources, home health resources, therapeutic counseling, and post-hospital support to optimize outcomes. Works collaboratively with the Care Coordinator and other patient care team members. Serves as an advocate to ensure that the patient's physical, psychological, and social needs are identified and fulfilled on a timely basis and that rights are respected and addressed.

Licensure
