Job Title: Case Manager, Scattered Site Housing

Hours:Monday-Friday 8:00am-4:30pm

SUMMARY
As part of a multi-disciplinary team, the Intensive Case Management Services (ICMS) Case Manager will travel to multiple locations in the greater Los Angeles area to meet and work with clients to attain and sustain permanent housing in partnership with the County Department of Health Services (DHS). The ICMS Case Manager will work with clients to assess housing stability, provide stabilization services, and coordinate multiple services to meet each client’s specific needs related to housing, health, mental health, and income.
