Job summary

EAC Network in Deer Park, NY seeks a full-time Case Manager to support youth and families, conducting interviews with DSS-sanctioned clients, collecting data, and coordinating referrals to labor and DSS services. The role requires a high school diploma or GED, at least one year in human services, strong communication and computer skills; reliable car and NY license are preferred.

Qualifications

• High School Diploma or GED required; degree in social/behavioral sciences preferred.
• Minimum one year of experience in human services.
• Strong communication, both oral and written, and computer skills.
• Access to automobile and NYS driver's license.

Responsibilities

• Conduct interviews of DSS-sanctioned clients and collect data.
• Enter assessment information into the database and coordinate referrals.
• Refer clients to DSS Employment Management Program or Dept. of Labor as indicated.
• Travel to DSS sites as needed to support program operations.
