Case Manager - RHY

The Door is an unparalleled model for youth development, offering a comprehensive range of integrated services within a single site for nearly eleven thousand New York City youth each year. Our mission is to empower young adults to reach their full potential by providing comprehensive youth development services in a diverse and caring environment. At The Door, youth can access health care and education, mental health counseling and crisis assistance, legal assistance, college preparation services, career development, housing supports, arts, sports and recreational activities, and nutritious meals – all for free and under one roof. By providing participants with our suite of integrated services, we seek to provide any motivated young person with the tools, resources, and opportunities needed to successfully transition to adulthood.

The Case Manager will support the Runaway and Homeless Youth program by providing case management to homeless and unstably housed youth to ensure that their basic needs—including crisis counseling, housing placement, entitlements procurement, health care, mental health care, education, job/career assistance, recreational opportunities, and other service needs—are met.

Responsibilities:
  1. Identify young people in need of case management services in collaboration with other Door programs.
  2. Provide supportive counseling and case management.
  3. Participate in case conferences for optimal collaboration among departments, programs, and external agencies.
  4. Perform fieldwork and escorts as required.
  5. Assist with making shelter referrals for youth/young adults and provide follow-up as needed.
  6. Document all interventions with youth/young adults.
  7. Demonstrate understanding and practice of positive youth development principles.
  8. Attend relevant meetings, trainings, workshops, and outside events.
  9. Develop and facilitate groups for youth/young adults as needed.
  10. Perform ACS mandated reporting duties as needed.
  11. Provide suicide/homicidal assessments in collaboration with supervisor as needed.
  12. Assist the RHY team in providing basic needs services, such as food, clothing, and shower/laundry services.
  13. Handle any other responsibilities assigned by supervisor.
Qualifications:
  • Bachelor’s degree required.
  • 1-2 years of direct service case management experience with urban homeless youth and LGBTQ populations.
  • Knowledge of and ability to apply positive youth development principles.
  • Experience working with adolescents and other city agencies.
  • Ability to engage, advocate, and collaborate on behalf of clients.
  • Effective interpersonal and organizational skills.
  • Proficiency in verbal, written, and computer skills.
  • Proficiency in Spanish, French, or Mandarin preferred.
Work Schedule:

Full-time, Wednesday - Sunday, 7:00 am - 3:00 pm, 35 hours/week

Salary:

$46,000 - $48,000, plus generous paid time off and 1199 Union Benefits
