Job summary

Covenant House New York is seeking a proactive Rights of Passage (ROP) Case Manager to support 10-20 young people aged 16-21 living in a Bronx site. You will develop individual service plans, coordinate services, provide counseling, advocacy, and monitor progress within a dedicated multidisciplinary team.

The role requires strong group leadership, resilience, and trauma-informed approaches, with willingness to work evenings, weekends, and holidays.

Qualifications

• High School or GED required with one year of residential youth experience preferred.
• Experience with resiliency, strength-based youth development, and trauma-informed care.
• Ability to manage multiple projects and take initiative in a team setting.

Responsibilities

• Provide integrated counseling and case management to engage youth and promote wellness, housing stability, and service-plan goals.
• Develop and monitor an individual service plan with youth and coordinate services with internal/external partners.
• Meet with youth regularly to track progress toward ISP goals and adjust plans as needed.
• Advocate for youth with external service providers and collateral services.
• Maintain confidential records, charts, and logs; ensure regulatory compliance and proper documentation.
• De-escalate crises and respond to emergencies following agency protocol.

About the role

The Rights of Passage (ROP) Case Manager provides supportive services to 10-20 young people aged 16-21 living in a ROP site in the Bronx. Reporting to the Case Manager Coordinator of the ROP site, the Case Manager is an energetic, creative, and diligent member of a team that includes resident advisors (who also have cooking responsibilities) and a cadre of specialty staff, including employment/education specialists, housing specialists, youth development staff, and mental health staff. The Case Manager will provide comprehensive case management services to the youth who are on their caseload, including the creation of an individual service plan, coordination of services, individual counseling, advocacy, and monitoring of the youth’s efforts and challenges in completing goals and celebrating goal achievement.

What you'll do

• Provides integrated counseling and case management services that effectively engage young adults and promote personal growth and wellness, housing stability, and ongoing accomplishment of service plan goals.
• Collaborates with youth to assess strengths and challenges and develop an individual service plan that reflects the youth’s progress and stated goals.
• Meets with youth regularly to determine progress with goals identified in the ISP.
• Advocates on behalf of youth with outside service providers and other collateral services.
• Creates and maintains confidential records, charts, and logs for youth, documenting all relevant information from individual and group interventions and with collateral services.
• Ensures that youths’ files are compliant with all regulatory and other requirements.
• De-escalates crises; attends to emergencies following agency protocol, as needed.

Qualifications

• If no Bachelor's degree is required, High School or GED is required, with one (1) year of experience in a residential setting working with adolescents preferred.
• Strong skills in leading groups, taking the lead in collaborating with internal and external departments
• Demonstrated knowledge of and experience working directly with young people through approaches emphasizing resiliency, strength-based youth development, and trauma-informed care required.
• Strong multi-tasking skills with demonstrated ability to manage multiple projects at once required.
• Excellent verbal, written, and problem-solving abilities and an upbeat demeanor are required.
• A strong focus on child protection and ensuring the safety of residents is required.
• Experience working with diverse groups is required.
• Overall flexibility in work schedule required, including weekends, evenings, and holidays if needed.
• Experience utilizing evidence-based and other social service models preferred.
