Universal Health Services (UHS) is seeking a Case Manager to lead multidisciplinary planning and coordinate care across the hospital continuum in Bradenton, FL. The role focuses on organizing patient care sequences to maximize outcomes and efficiency, while collaborating with physicians, nurses, and payers.

Ideal candidates will have a Bachelor’s degree, at least two years of social work experience, and a preference for care management certification.

Utilizing a team approach, this position coordinates and organizes multiple strategies to ensure patients are managed with minimal overlapping roles and processes. Collaborates with physicians, nurses, other case managers, direct and indirect providers, community resources, and payers.
