The Case Manager (CM) ensures high-quality, patient-centered rehabilitative care, managing patient needs to achieve optimal outcomes. Responsibilities include providing program orientation, coordinating patient and family education, implementing treatment plans, documenting assessments, and promoting positive patient interactions.
Qualifications include a current Florida RN license or appropriate compact license, a diploma or related healthcare program, at least 3 years of recent acute clinical experience, and strong critical thinking and communication skills. Knowledge of healthcare resources and governmental/private organizations is essential.
