RWJBarnabas Health is seeking a highly dedicated RN to work per diem in Case Management for Monmouth Medical Center.

Qualifications:

Required:

• BSN required.
• NJ RN License
• 3 to 5 years acute care RN experience
• Case Management experience

Preferred:

• Utilization Review or Discharge Planning Experience
• Case Management Certification

Scheduling Requirements:

• 8-4:30 or 9-5

Essential Functions:

• The Case Manager coordinates quality Healthcare services to meet our patient s needs & assures they are generated in a timely and cost effective manner & helps them and their family make informed Healthcare decisions.
