Job summary

Beth Israel Lahey Health is seeking a Registered Nurse Case Manager in Massachusetts to coordinate patient care, assess needs, and facilitate appropriate utilization and discharge planning.

Graduates of accredited nursing programs with MA RN licensure are preferred, with BSN and Case Management certification highly desired. 2–3 years of CM experience is preferred; the role operates under a collective bargaining agreement. On-site hospital-based position.

Qualifications

• Experience in care coordination and discharge planning.
• Knowledge of regulatory compliance and utilization management.
• Familiarity with working under a collective bargaining agreement.

Responsibilities

• Coordinate discharge plans between care teams, patients, families, insurers and continuation of care agencies.
• Perform concurrent and retrospective reviews to ensure appropriate utilization and prevent denials.

Job Description:

Case managers serve as the care coordinators for patients, care givers, and families. Their functions include concurrent and retrospective review toensureappropriate utilization, assessment, prevention ofdenials,and coordination of discharge options. Theyare required tofollow specific criteria sets toensureregulatory compliance. They coordinate safe discharge plans between the health care delivery team, patients, families, insurers, and continuing care agencies.

Education/Experience:

Graduate of an accredited school of nursing

BSN preferred

2-3 years of Case Management experience preferred

Certification/License:

Current Massachusetts RN licensure

Certification in Case Management preferred

Pay Range:

$36.00 - $79.12

This position is covered by a collective bargaining agreement. The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. The pay range for this position is established and governed by the terms of the applicable collective bargaining agreement. Employees hired into this position will receive compensation consistent with the terms of the applicable collective bargaining agreement.
