The Case Manager RN utilizes advanced nursing skills to coordinate care for a specific patient population, facilitating their journey through the continuum of care.
Work Schedule: M-F 8:30-4:30, with 1 weekend day every 4 weeks, 1 major holiday, and occasional floating to all 3 campuses.
Required: BSN, 2 years of medical-surgical nursing experience.
Preferred: 1-2 years of acute care, home care, or population health RN case management experience, CCM certification.
$81,429.00 to $116,036.80. Actual hiring rate depends on experience, skills, and qualifications. This position is not eligible for an annual incentive.
Work Shift: Day shift (United States). Worker Sub Type: Regular. Location: 435 Hurffville-Cross Keys Rd, Turnersville, NJ.
