Job summary

Saint Francis Hospital in Colorado Springs is seeking a Case Manager to join Lifepoint Rehabilitation. The role emphasizes patient assessments, care planning, and interdisciplinary collaboration to improve community health outcomes. Day shift; minimum 24 hours/week, up to 36 hours.

We value nursing licensure, inpatient CM experience, and CM certification credentials. Strong English communication is required; bilingual skills are a plus.

Qualifications

• Current RN or licensed healthcare professional preferred.
• Certification in Case Management or Rehabilitation Nursing preferred (CCM/ARN/ACM/RN-BC).
• 2+ years social work or case management in inpatient setting preferred.
• Excellent English communication; additional languages a plus.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides care appropriate to condition and age of the patient.
• Documents medical necessity in the medical record in a timely manner.
• Responsible for completion and revision of the Interdisciplinary Care Plan.
• Performs timely and accurate QI assessments.

How You'll Contribute

What We're Looking For

• We prefer a current Registered Nurse or Healthcare professional licensure as Physical Therapist, Speech Language Pathologist or Occupational Therapist
• We prefer a Certification in Case Management or Rehabilitation Nursing for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• Effective oral and written communication skills in English, additional languages preferred.

Hourly range: Salary range: $44-45 per hour.
