Valley Children\'s Healthcare in California is seeking a nurse in a case management role to assess, plan, coordinate, and evaluate patient care across the continuum. You will work with the health care team to ensure appropriate utilization and timely services for a specified group of patients.

The position requires an active CA RN license, a two-year nursing degree (2-year) and a four-year degree in a related field is preferred, and at least two years of pediatric nursing experience; bilingual

Job Summary:

This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet a patient's health needs. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient's clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act, and in accordance with the policies on Valley Children's Healthcare.

Qualifications:

Education

• College Degree - 2 year Nursing (required)
• College Degree - 4 year Nursing or in a field that supports the Case Management's process (preferred)

Licenses and Certifications

• RN - Registered Nurse License - CA-BRN - California Board of Registered Nursing (required)

Work Experience

• Minimum two (2) years Nursing experience (required)
• Pediatric experience and Case Management/Utilization Management experience desirable. (preferred)

Skills and Abilities

• Bilingual Skills Preferred.
