Job summary

Sinai Hospital, part of LifeBridge Health, seeks a Case Manager to coordinate discharge planning and transitional care for medically complex patients. You will work with the clinical team to promote safe transitions and reduce denials while improving outcomes. This 12-hour Day shift operates Saturdays and Sundays.

Required is a Nursing Degree with an active Maryland RN license; 3–5 years in inpatient case management is preferred, with CM certification a plus. Competitive benefits are offered.

Qualifications

• 3-5 years related experience; inpatient case management/discharge planning preferred.
• RN license; Maryland license or eligibility to obtain Maryland license.
• Case Manager Certification within 3-5 years of hire is preferred.
