Job summary

Amedisys in Knoxville, TN seeks an experienced RN Case Manager to coordinate home health care in the Oak Ridge area. You will assess patients, develop plans of care with the care team, and support families in a home setting.

Qualifications

• One year of nursing experience.
• Current RN license for the assigned state(s).
• Current CPR certification.
• Valid driver's license and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
