Job summary

Amedisys is seeking an RN Case Manager to cover the Oak Ridge area, joining one of the nation's leading home health and hospice providers. This role focuses on patient assessments, care planning, and collaboration with the care team to support healing at home.

Qualifications

• At least 1 year of nursing experience.
• Current RN license for assigned state(s).
• CPR certification required.
• Valid driver’s license with reliable transportation.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness.
• Record pain/symptom management changes/outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Regularly communicate with the clinical manager and care team.
• Perform on-call responsibilities and on-call services as assigned.
• Other duties as assigned.
