Job summary

Seattle Children’s is seeking a dedicated nurse to coordinate care across patient populations from admission through discharge. You will advocate for patients, collaborate with families and the health care team, and promote efficient resource use and timely discharge planning.

The role requires a BSN degree and a WA nursing license, with a minimum of three years of clinical experience. BLS is required, with PEARS/PALS accepted as substitutes, and ongoing education support is provided.

• Current Washington State Nursing License.
• Current BLS for Healthcare Providers is required for all positions; PEARS/PALS accepted in lieu of BLS.
• ACLS will not be accepted as a substitute.
• PALS certification may be required for specific departments.
• Case Managers in Child Psych Consultation must obtain Safety Care Certification within 3 months of hire and annually thereafter.

Job description

1.0 FTE, 40hrs per week, 8hr shifts. Start time 630AM, 7AM or 730AM.

Has responsibility, in collaboration with the health care team and family, for the coordination of care for specific patient populations from admission through discharge. This includes advocacy, accountability for the achievement of identified outcomes in a timely manner, promotion of collaborative practice, appropriate use of resources, facilitation of timely discharge, coordination of discharge planning, resource acquisition, and patient/family education. This is accomplished through initial and ongoing assessment, effective participation in the care planning process, and collaboration with the team and community providers. Supports staff/physician education related to the care coordination process.

Required Education And Experience

BSN degree or equivalent combination of education and experience.

Minimum Of Three(3) Years Of Recent Clinical Experience

Ability to communicate in a second language may be required for specific departments.

Required Credentials

Current Washington State Nursing License.

At time of hire: Current Basic Life Support (BLS) for Healthcare Providers is required for all positions.

Pediatric Emergency Assessment, Recognition and Stabilization (PEARS), and Pediatric Advanced Life Support (PALS) will be accepted in lieu of BLS for Healthcare Providers.

As a condition of employment: Current BLS for Health Care Providers is to be maintained at all times by all nurses.

Note: current PALS and/or PEARS will be accepted in lieu of BLS, and must be maintained as current at all times.

ACLS will not be accepted as a substitute to the requirements listed above.

PALS certification may be required for specific departments.

Case Managers working in Child Psych Consultation are required to obtain Safety Care Certification (internal training) within 3 months of hire and annually thereafter.

Preferred

BSN/MS/MSN degree.

Minimum Of Five (5) Years Of Recent Clinical Experience

Pediatric clinical expertise in area of responsibility.

Experience in continuity of care/discharge planning.

Compensation Range

$51.60 - $91.30 per hour

Salary Information

This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
