Registered Nurse (RN) - Traveling Role

Location: Allegheny County, PA

Schedule: Monday-Friday, 8:00 AM - 5:00 PM

Duration: 4-7 months

Position Overview

We are seeking a dedicated and experienced Registered Nurse (RN) to provide care across five skilled nursing facilities (SNFs) in Allegheny County: Woodhaven, Harmar Village, Riverside, East End, and Squirrel Hill. This role is part of the Institutional Special Needs Program (ISNP) and is essential in addressing staffing shortages. The RN will work closely with interdisciplinary teams to deliver high-quality care for approximately 115 members while traveling between facilities during their shift.

Primary Responsibilities

• Ensure accurate and complete documentation of patient care
• Collaborate with the interdisciplinary care team to optimize patient outcomes
• Assist providers with various care coordination activities
• Support benefit determination related to Medicare Part A, Part B, and Part D
• Monitor and respond to patient changes in condition, collaborating with the APC and facility teams
• Coordinate therapy and rehabilitation services
• Attend and participate in facility-based meetings
• Schedule and lead family conferences, team meetings, and case presentations
• Manage training, tracking, and compliance with quality measures
• Conduct in-service training sessions for staff
• Assist providers with documentation, coding, medication reconciliation, nursing rounds, assessments, and charting
• Write verbal orders in compliance with RN practice laws and facility regulations
• Provide acute care and assist in chronic condition management
• Support short-term sub-acute rehabilitation episodes
  
  

Required Qualifications

• Active and unrestricted RN license in Pennsylvania or ability to obtain before start date
• Associate degree or higher in Nursing
• Basic Life Support (BLS) certification
• Ability to perform physical assessments and lift up to 30 pounds
• Reliable transportation to travel between facilities during shifts
• Full COVID-19 vaccination required per company and regulatory guidelines
• Must pass a drug screening before employment
  
  

Preferred Qualifications

• Minimum 1 year of clinical experience (long-term care setting preferred)
• Experience managing patients with complex behavioral, social, and functional needs
• Strong understanding of geriatrics and chronic illness care
• Proficient computer skills, including electronic medical record (EMR) documentation
• Excellent communication and time management skills
• Ability to work effectively with diverse care teams in various settings
