Job summary

TriStar Centennial Medical Center, part of HCA Healthcare, is seeking a Case Manager RN in our Rehabilitation program to guide patients and families through the healing process with high-quality, personalized care.

As an RN, you will coordinate education, develop and implement treatment plans, complete psychosocial assessments, and collaborate with the Rehab Program Director and care team to ensure seamless, patient-centered outcomes.

Qualifications

• RN licensed registered nurse
• Associate degree, baccalaureate degree, or RN diploma required or equivalent
• Experience in case management or rehabilitation setting preferred

Responsibilities

• Guide patients and families through program orientation and rehab philosophy, discharge criteria, and patient rights
• Coordinate education for patients, families, and caregivers to support participation in treatment planning
• Develop and implement individualized treatment plans reflecting patient strengths and goals
• Complete psychosocial assessments and discharge planning evaluations and document findings
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care
• Promote compassionate, respectful, and excellent care across interactions

Job Summary and Qualifications

At HCA Healthcare, we care like family. As a Case Manager in our Rehabilitation program,you’llplay a vital role in guiding patients and families through the healing process — ensuring each patient receives high-quality, personalized care that supports recovery, independence, and hope.

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction
