Job summary

TriStar Centennial Medical Center is seeking a Case Manager RN to guide patients and families through rehab pathways, discharge planning, and care coordination. You will assess psychosocial needs, educate patients and families, and collaborate with the Rehab Program Director and care team to ensure seamless, patient-centered recovery.

Qualifications

• Must hold a valid RN license.
• Degree in nursing from an accredited program.

Responsibilities

• Guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans reflecting patient strengths, needs, and recovery goals
• Completing psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team
• Collaborating with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promoting HCA Healthcare values through every patient and family interaction

Job Summary and Qualifications

Your responsibilities will include:

What qualifications you will need:
