The Case Management RN at Ahhmgt in Louisiana leads the development, planning and coordination of Utilization Review and Resource Management to ensure high-quality, cost-efficient patient care across the facility. Daily responsibilities include chart reviews to determine medical necessity for admission and continued stay, coordinating discharge planning and interfacing with care teams to optimize outcomes.
