Job summary

Jobtailor is seeking a skilled nurse to coordinate care and discharge planning for a defined patient population. You will advocate for patients and families, monitor progress, and intervene when needed to keep care on track.

The role requires a Bachelor’s in Nursing and at least 3 years in case management or discharge planning, with strong conflict resolution and documentation abilities. Leadership will escalate barriers as needed.

Qualifications

• Bachelor's Degree of Nursing is required.
• Minimum 3 years in case management or discharge planning.
• Demonstrates effective conflict resolution skills.
• Demonstrates strong prioritization and time management.
• Applies medical necessity criteria for all patients.
• Precept and mentor new staff to their role and department.
• Documents all relevant information in the Medical Record per policy.
