Job summary

HCA Florida Oak Hill Hospital is seeking a Registered Nurse Case Manager to coordinate care, manage length of stay, and develop a safe discharge plan with the multidisciplinary team. The RN CM will facilitate progression and transition of care and promote quality outcomes.

You will collaborate with physicians, social services, and care teams to optimize outcomes, educate patients and families, and ensure resource utilization for inpatient and observation patients.

Qualifications

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ years in case management or 3+ years in clinical nursing required.
• Active RN license in applicable state.
• InterQual experience preferred; Case Management certification preferred.

Responsibilities

• Provide case management services for inpatient and observation patients.
• Identify patients at risk for adverse outcomes during transitions.
• Perform comprehensive psychosocial and discharge assessments.
• Reassess and adjust care plans as indicated.
• Coordinate discharge planning with multidisciplinary team and physicians.
• Collaborate with Social Services to ensure appropriate post-acute care.

Job Summary And Qualifications

The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

Your job responsibilities will include:

• Provide case management services for both inpatient and observation patients as assigned.
• Identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another.
• Perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family.
• Reassess the patient’s clinical condition as indicated.
• Consider patient’s readmission status or risk of readmission and develop strategies to mitigate including education on appropriately accessing healthcare resources, preventative education, and community based resources.
• Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition.
• Partner with Social Services to ensure the post-acute medical needs and level of care are appropriate.

What qualifications you will need

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ year experience in case management OR 3+ years’ experience in clinical nursing required.
• Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation required.
• InterQual experience preferred
• Certification in Case Management preferred
• Ability to establish and maintain collaborative and effective working relationships
• Ability to communicate effectively in oral, written and electronic formats
• Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills
