Position Summary:

Sign-on bonus potential up to $17,500 for candidates with valid NJ Choice Certification, or $7,500 for other eligible external candidates. This is a remote role with 50-75% field travel within Union County and surrounding areas. The schedule is Monday - Friday, 8 am - 5 pm EST.

The role involves developing and supporting health service strategies, policies, and programs that ensure quality healthcare delivery. The position requires advanced clinical judgment and critical thinking to facilitate appropriate physical, behavioral health, and psychosocial services. Responsibilities include care planning, provider collaboration, and resource utilization in a cost-effective manner. Strong assessment, communication, and documentation skills are essential.

Key Responsibilities:

• Conduct face-to-face visits in members' homes, utilizing assessment tools for members enrolled in MLTSS and/or D-SNP/FIDE programs.
• Evaluate medical needs of non-MLTSS members to promote wellness and facilitate service access.
• Coordinate and collaborate care with members, providers, and care teams.
• Develop care plans and authorize services within the MLTSS/FIDE benefits, documenting accurately and timely.
• Mentor new hires once proficiency is attained.

Qualifications:

• Must reside in Union County.
• 3+ years of clinical practice experience.
• Active and unrestricted RN license in NJ.
• Successful completion of NJ Choice certification (exam score of 80%+ required).
• Own transportation is required.

Preferred Qualifications:

• Experience with LTSS, discharge planning, managed care, Medicaid, and Medicare.
• Proficiency in computer applications including MS Office Suite.

Education:

• Associate degree or Diploma in Nursing minimum.

Compensation and Benefits:
