Case Manager, Registered Nurse (Montgomery, Berks, Bucks, Dauphin, Delaware, and Philadelphia Counties, Pennsylvania)

Position Summary

Schedule: Monday through Friday, 8:00 AM - 5:00 PM EST

Travel: Up to 10-25% within Montgomery, Berks, Bucks, Dauphin, Delaware, and Philadelphia Counties

Join our Community Care team to provide community-based care management, collaborating with members, providers, and community organizations to address health and social needs. Facilitate benefits delivery, develop health strategies, and support member wellness and timely return to work.

Responsibilities
  • Act as liaison with members, families, providers, insurers, and healthcare personnel.
  • Coordinate case management for catastrophic and chronic cases, including referrals and home visits.
  • Assess and develop care plans for members’ medical and vocational needs.
  • Communicate with stakeholders and document case activities.
  • Consult with multidisciplinary teams and healthcare providers.
  • Provide educational and preventive health information.
  • Ensure compliance with laws, regulations, and insurance requirements.
  • Evaluate needs and facilitate appropriate care and benefits.
  • Monitor progress and develop proactive interventions.
Qualifications
  • Active Pennsylvania RN license.
  • Minimum 3 years clinical experience, preferably with Medicare populations.
  • At least 2 years in case management, discharge planning, or home health.
  • Willing to travel 10-25% within specified counties.
Preferred Skills and Certifications
  • Strong analytical, communication, and organizational skills.
  • Ability to work independently, possibly remotely.
  • Proficiency in MS Office and proprietary systems.
  • Certified Case Manager preferred.
  • Additional certifications (CRC, CDMS, CRRN, COHN, CCM) advantageous.
Education

Associates degree or Nursing Diploma required; Bachelor of Science in Nursing preferred.

Additional Details

40 hours per week, full-time position. Salary range: $60,522.80 - $129,615.20, commensurate with experience and qualifications. Benefits include medical, 401(k), stock purchase plan, wellness programs, paid time off, flexible schedules, and more.
