This position with Aetna’s Long-Term Services & Support (LTSS) team is a field-based role originating from Danville, Bristol, or the Southwest Region of Virginia. Candidates must hold a current, unrestricted Virginia RN, LPN, or BH license and be willing to travel up to 50-75% to meet with members.

The Nurse Case Manager assesses, plans, implements, and coordinates case management activities to evaluate members' medical needs and facilitate overall wellness. Responsibilities include developing proactive strategies to improve short- and long-term health outcomes, utilizing clinical tools and data review, and ensuring smooth transitions to Aetna programs and plans.

Key duties involve applying clinical judgment to reduce risk factors and barriers, addressing complex health and social issues, reviewing prior claims, and assessing functional capacity. The role requires a holistic approach, including referrals to clinical resources and collaboration with supervisors and multidisciplinary teams.

The position involves performing detailed document inspections, using a PC, and engaging with members via telephone and in a sedentary work environment.
