This Case Manager Registered Nurse position is with Aetna's Long-Term Services and Supports (LTSS) team and is a field-based role based in the Tidewater Region Virginia, specifically the Norfolk Area. Candidates must hold a Virginia LPN, RN, or BH License and be willing to travel 50-75% of the time to meet with members face-to-face.
Typical office environment with productivity and quality expectations. Work involves close inspection of handwritten and computer-generated documents, as well as working at a PC monitor. It requires sitting for extended periods, talking on the telephone, and typing on the computer.
The Nurse Case Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate overall wellness. This includes developing proactive strategies to address issues, utilizing clinical tools, reviewing data, and ensuring smooth transitions to Aetna programs. The role involves applying clinical judgment to reduce risk factors, address complex health and social indicators, and coordinate care plans that consider co-morbidities and functional status. Collaboration with supervisors and multidisciplinary teams is essential to overcome barriers and meet member needs.

Time type: Full-time
Pay range: $60,522.00 - $129,615.00, depending on experience, education, and location. The role may include bonuses or incentive programs.
