Case Manager Registered Nurse (Field Must reside in Southern and Lower Central Region of New Jersey)

This position is with Aetna's Integrated Care Management (ICM) team and is field-based out of the Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County. Candidates must hold a NJ Registered Nurse License and be willing to travel 50-75% of the time to meet members face-to-face.

The care manager assesses and evaluates members with potential care needs through telephonic and face-to-face assessments in various settings, including members' residences, hospitals, behavioral health, and long-term care facilities. They develop and monitor cost-effective, member-centric care plans in collaboration with members, caregivers, and providers, adjusting plans based on clinical judgment and member needs. Coordination with the care team ensures timely service delivery, and addressing social needs such as housing and food insecurity is part of the role. Accurate documentation in electronic health records is essential. Critical thinking, problem-solving skills, and the ability to work autonomously are required. Mentoring new hires may be part of the responsibilities.

Required Qualifications
  • Must reside in Southern and Lower Central Region of New Jersey; preferably Ocean, Monmouth, or Atlantic County
  • 3+ years' experience with Pediatric, Special Needs, or Adult populations
  • 2+ years managing populations with mental health and/or substance use disorders
  • RN with current, unrestricted New Jersey license
  • Reliable transportation and willingness to travel 50-75%; mileage reimbursed
  • Proficiency in computer systems and MS Office Suite
Preferred Qualifications
  • Certified Case Manager Certification
  • Experience in an integrated case management model
  • Bilingual skills
  • Strong communication, problem-solving, and decision-making skills
  • Knowledge of medical terminology and digital literacy
  • Ability to handle sensitive information ethically
Education
  • Bachelor's degree in Science & Nursing
Additional Details

Full-time, 40 hours/week. Salary range: $72,627 - $155,538, depending on experience and location. Benefits include medical, 401(k), stock purchase, wellness programs, paid time off, flexible schedules, and more. Application deadline: 06/30/2025.
