Case Manager Registered Nurse - Field Must reside in Northern and Upper Central Region of New Jersey; Preferably Mercer, Middlesex, or Somerset County - $1,000 Sign on Bonus

Join CVS Health as a Case Manager Registered Nurse with a $1,000 sign-on bonus. This position is with Aetna's Integrated Care Management (ICM) team, based in the Northern and Upper Central Region of New Jersey, preferably in Middlesex, Bergen, or Essex County.

The role involves assessing and evaluating members' care needs through telephonic and face-to-face assessments in various settings, including members' homes, hospitals, behavioral health, and long-term care facilities. Requirements include holding an NJ RN license, residing in the specified region, and being willing to travel 50-75% of the time.
