Case Manager Registered Nurse (Field – Must reside in Northern and Upper Central Region of New Jersey; Preferably Mercer, Middlesex, or Somerset County)

Position Summary

This Case Manager Registered Nurse position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based role out of the Northern and Upper Central Region of New Jersey, preferably Middlesex, Bergen, or Essex County. Candidates must hold an NJ Registered Nurse License and be willing to travel 50-75% of the time for face-to-face member meetings.

The care manager assesses and evaluates members with potential care management needs through telephonic and face-to-face assessments in various settings, including private residences, hospitals, behavioral health facilities, and long-term nursing facilities.

They establish a cost-effective, member-centric care plan in collaboration with the member, authorized caregivers, and providers. The care manager monitors and evaluates the effectiveness of these plans, adjusting based on clinical judgment and member needs. They coordinate with members, authorized representatives, primary care providers, and other care team members to ensure timely service delivery and advocate for members’ needs, including post-hospitalization discharges and transitions from nursing facilities, addressing social needs like housing and food insecurity. Accurate, timely documentation in the electronic health record is essential. The role requires critical thinking, problem-solving skills, and autonomy. Mentoring new hires may be expected once proficient.

Required Qualifications
  • Must reside in the Northern and Upper Central Region of New Jersey, preferably Middlesex, Bergen, or Essex County
  • Reliable transportation and willingness to travel 50-75% with mileage reimbursement
  • 3+ years’ experience with Pediatric, Special Needs, or Adult populations
  • 2+ years managing populations with mental health/substance use disorders
  • RN with current unrestricted New Jersey license
  • Proficiency in navigating computer systems and MS Office applications
Preferred Qualifications
  • Certified Case Manager Certification
  • Experience in an integrated case management model
  • Bilingual skills (preferred)
  • Effective verbal and written communication skills
  • Problem-solving and decision-making skills
  • Knowledge of medical terminology and digital literacy
  • Ability to handle sensitive information ethically and tactfully
Education
  • Bachelor’s Degree in Science & Nursing
Additional Information

Anticipated weekly hours: 40

Time Type: Full-time

Pay Range: $72,627.00 - $155,538.00, depending on experience and other factors. Role eligible for bonus and incentive programs.
