Position Summary

This position offers a $1,000 Sign on Bonus for external applicants! The Case Manager Registered Nurse position is with Aetna's Integrated Care Management (ICM) team and is a field-based role out of the Northern and Upper Central Region of New Jersey, preferably Middlesex, Bergen, or Essex County. Candidates must hold an NJ Registered Nurse License and be willing to travel 50-75% of the time for face-to-face member meetings.

The care manager assesses and evaluates members with potential care needs through telephonic and face-to-face assessments in various settings, including the member's residence, hospitals, behavioral health, and long-term care facilities. They develop a member-centric, cost-effective care plan in collaboration with the member, caregivers, and providers. The role involves monitoring and adjusting care plans based on clinical judgment and member needs, coordinating with care teams, and advocating for member needs to ensure safe discharges and transitions, including addressing social determinants like housing and food security. Accurate documentation in electronic health records is essential. Critical thinking, problem-solving, and autonomous work are required, with potential mentorship responsibilities for new hires.

Required Qualifications
  • Must reside in the Northern and Upper Central Region of New Jersey, preferably Middlesex, Bergen, or Essex County
  • Reliable transportation and willingness to travel 50-75% for face-to-face meetings (mileage reimbursed)
  • 3+ years' experience with Pediatric, Special Needs, or Adult populations
  • 2+ years managing populations with mental health/substance use disorders
  • RN with current unrestricted NJ license
  • Proficiency in computer systems and MS Office applications
Preferred Qualifications
  • Certified Case Manager Certification
  • Experience in an integrated case management model
  • Bilingual skills
  • Strong communication, problem-solving, and decision-making skills
  • Knowledge of medical terminology and digital literacy
  • Ability to handle sensitive information ethically
Education

Additional Details

Full-time, 40 hours/week, with a pay range of $72,627 - $155,538, dependent on experience and location.
