Case Manager Registered Nurse (Field - Must reside in Middlesex, Bergen or Essex County New Jersey)

Join CVS Health as a Case Manager Registered Nurse (Field - Must reside in Middlesex, Bergen or Essex County New Jersey).

This role is with Aetna’s Long-Term Services and Supports (LTSS) team, based in Northern New Jersey, preferably Middlesex, Bergen, Essex, Somerset, or Union County. Candidates must hold a NJ Registered Nursing License and be willing to travel 50-75% to meet members face-to-face.

Position Overview

The care manager assesses and evaluates members' care needs through telephonic and face-to-face assessments in various settings, including homes, hospitals, behavioral, and long-term care facilities. Responsibilities include developing cost-effective, member-centric care plans, monitoring their effectiveness, and adjusting them as needed. The role involves coordinating with members, caregivers, and healthcare providers, advocating for members' needs, addressing social determinants of health, and documenting all activities accurately in electronic health records. Critical thinking, problem-solving, and autonomous work are essential. Mentoring new hires may be part of the role.

Qualifications

• 3+ years’ experience with Pediatric, Special Needs, or Adult populations
• RN license in New Jersey (unrestricted)
• Reside in Middlesex, Bergen, Essex, Somerset, or Union County, NJ
• Reliable transportation and ability to travel 50-75%
• Proficiency in computer systems and MS Office Suite

Preferred Skills

• 2+ years managing mental health/substance use disorder populations
• Certified Case Manager Certification
• Experience in an integrated case management model
• Bilingual abilities
• Strong communication, problem-solving, and medical terminology knowledge

Additional Details
