Position Summary
Job description:
Develop, implement, support, and promote health service strategies, tactics, policies, and programs that drive the delivery of quality healthcare to our members. Health service strategies, policies, and programs include utilization management, quality management, network management, clinical coverage, and policies. The position requires advanced clinical judgment and critical thinking skills to facilitate appropriate physical, behavioral health, and psychosocial wrap-around services. The care manager will be responsible for care planning, direct provider collaboration, and effective utilization of available resources in a cost-effective manner. Strong assessment, writing, and communication skills are required.
Fundamental Components/Job Description:
The Case Manager is responsible for conducting face-to-face visits in the members' homes using comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Program (D-SNP/FIDE). The case manager may also assess non-MLTSS members to evaluate medical needs and facilitate overall wellness, addressing requests such as adult or pediatric medical daycare, personal care, nursing facility requests, and program enrollments. Completion of the NJ Choice Certification is required for employment. Member assignments may include pediatric and medically complex cases. The case manager coordinates care with members, authorized representatives, PCPs, and care teams, schedules interdisciplinary meetings, advocates for safe discharges, develops care plans, and authorizes services within the MLTSS/FIDE benefits. Accurate and timely documentation in the electronic health record is essential. The role requires critical thinking and problem-solving skills. Mentoring new hires is also expected after proficiency is attained. Travel of 50-75% within Atlantic, Cape May, and surrounding counties is required.

The typical pay range for this role is $72,627.00 - $155,538.00. The actual salary will depend on experience, education, location, and other factors. The position may include bonuses or incentives.
