About This Role

The Case Manager Registered Nurse role at CVS Health involves conducting in-person patient assessments and coordinating multidisciplinary care plans in a field-based setting. What You'll Do

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate the medical needs of members enrolled in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Program (D-SNP/FIDE).
• Facilitate member access to services such as adult medical daycare and personal care assistance.
• Schedule and attend interdisciplinary meetings to advocate for members.
• Collaborate with members and care teams to develop care plans.
• Authorize services in a cost-effective manner within MLTSS/FIDE benefits.
• Document accurately and timely in the member’s electronic health record.

Requirements

**Education:**

• Minimum associate’s degree or diploma in Nursing required.

**Experience:**

• 3+ years of clinical nursing experience required.

**Licenses & Certifications:**
