Qualifications

• Associate's degree required; Bachelor's degree preferred.
• 3–5 years clinical practical experience.
• 2–3 years in care management, discharge planning, and/or home health care coordination preferred.
• Active Illinois RN license required; Certified Case Manager preferred.

Requirements

• Education:

• Licenses & Certifications:
• Active and unencumbered Registered Nurse license in the state of Illinois required
• Certified Case Manager preferred
