About This Role

Requirements

Education:

Associate's degree required, bachelor's preferred

Experience:

• Minimum 3-5 years clinical practical experience
• 2-3 years Care Management, discharge planning, and/or home health care coordination experience preferred

Licenses & Certifications:

• Active and unencumbered Registered Nurse license in the state of Illinois required
• Certified Case Manager preferred
