Position Summary

This is a remote Case Manager opportunity with 50 to 75% field travel in Bergen County and surrounding counties. The schedule is Monday - Friday, 8 am - 5 pm EST. Develop, implement, and promote health service strategies, policies, and programs that ensure quality healthcare delivery. The role requires advanced clinical judgment, critical thinking skills, and involves care planning, provider collaboration, and resource utilization to support members' wellness and service needs.

Responsibilities

• Conduct face-to-face visits in members' homes, utilizing assessment tools for members in MLTSS and D-SNP/FIDE programs.
• Evaluate medical needs of non-MLTSS members to facilitate wellness and service access.
• Coordinate and collaborate with members, providers, and care teams.
• Develop care plans, authorize services, and document accurately in electronic health records.
• Mentor new hires after proficiency is attained.

Minimum Qualifications

• Must reside in Bergen County.
• 3+ years of clinical practice experience (hospital, home health, ambulatory care).
• Active RN license in NJ.
• Successful completion of NJ Choice Certification (score of 80%+ required).
• Own transportation required.

Preferred Qualifications

• Home health/hospice experience, LTSS, discharge planning, managed care, Medicaid/Medicare experience.

Education

• Associate degree or Diploma in Nursing minimum.

Additional Details

Pay range: $72,627 - $155,538, with potential signing bonus up to $10,000 for eligible external candidates. Full-time, 40 hours/week. Benefits include medical, 401(k), stock purchase plan, wellness programs, paid time off, flexible schedules, and more.
