Case Manager, Registered Nurse (Adams, Berks, Chester, Dauphin, or Franklin Counties, Pennsylvania)

Join us at CVS Health as a Case Manager, Registered Nurse in Adams, Berks, Chester, Dauphin, or Franklin Counties, Pennsylvania. This full-time role involves serving our community with compassion and expertise.

Position Summary

Schedule: Monday through Friday, 8:00 AM - 5:00 PM EST. Travel: Up to 10-25% within the specified counties.

As a Community Care Case Manager, you will assess, plan, facilitate, coordinate, evaluate, and advocate for our members' health and social needs, working collaboratively with members, providers, and community organizations to improve health outcomes.

Responsibilities

• Act as a liaison with members, families, providers, insurance companies, and healthcare personnel.
• Manage cases involving catastrophic and chronic conditions, including referrals, home visits, and use of community resources.
• Assess medical and vocational status; develop and implement care plans.
• Communicate with stakeholders and document all case activities.
• Coordinate with multidisciplinary teams and healthcare providers.
• Provide education and prevention information.
• Ensure compliance with applicable laws and regulations.
• Evaluate member needs and benefit eligibility, facilitating optimal health outcomes.

Qualifications

• Active Pennsylvania Registered Nurse (RN) license.
• Minimum 3 years clinical experience, preferably with Medicare populations.
• At least 2 years in case management, discharge planning, or home health care.
• Willingness to travel 10-25% within the specified counties.
• Preferred: Certified Case Manager, additional professional certifications, Bachelor's or Associate's degree in Nursing.

Compensation & Benefits

Pay range: $60,522.80 - $129,615.20 annually, based on experience and qualifications. Benefits include medical plans, 401(k), stock purchase, wellness programs, paid time off, flexible schedules, and more.
