Position Summary

Schedule: Monday through Friday, 8:00 AM – 5:00 PM EST. Travel: Up to 10-25% in Adams, Berks, Chester, Dauphin, or Franklin Counties, Pennsylvania.

You will have a life-changing impact on our Community Care members through a member-centric, team-delivered, community-based care management model that joins members where they are. With compassionate attention and excellent communication, we collaborate with members, providers, and community organizations to address health care and social determinant needs, aiming for quality, cost-effective outcomes.

Responsibilities include but are not limited to:

1. Facilitating delivery of benefits and healthcare information, promoting wellness activities, and developing/supporting health strategies, policies, and programs.
2. Using a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet health needs and promote wellness.
3. Acting as a liaison with members, families, employers, providers, insurance companies, and healthcare personnel.
4. Managing case activities for catastrophic and chronically ill members, including referrals, home visits, and community resources.
5. Interacting with members/clients telephonically or in person, including home or worksite visits.
6. Assessing medical and vocational status, developing care plans, and facilitating appropriate condition management.
7. Communicating with stakeholders, preparing documentation, and consulting with multidisciplinary teams.
8. Providing educational and preventive information, applying relevant laws/regulations, and testifying as needed.
9. Evaluating member needs, managing benefits eligibility, and monitoring progress toward outcomes.
