GENERAL STATEMENT OF RESPONSIBILITIES:
The Regional Mobile Crisis Response Case Manager Qualified Mental Health Professional (QMHP) provides mobile crisis response to people in crisis in their homes and other community settings within the catchment area listed above and the surrounding localities. Mobile crisis response includes assessing crisis situations and providing de-escalation and support for the consumer in crisis. The Case Manager will have ongoing support and clinical guidance from their supervisor. This position serves the assigned catchment area and will require the Case Manager to work from home, in the field, and at CSB offices to meet the needs of their CSB.
MAJOR DUTIES:
1. Provide immediate crisis supports to homes, and communities within assigned catchment area and surrounding localities across mental health, substance use, and intellectual/developmental disability issues.
2. Assist team members in the field.
3. Document all services and results in the consumer’s electronic health record by established deadlines.
4. Document progress and maintains a permanent record of the consumer’s activity according to established methods and procedures.
5. Oversee insurance and record keeping.
6. Communicate effectively and efficiently with Regional Mobile Crisis Program Manager(s).
7. Provide Regional Education Assessment Crisis Services Habilitation (REACH) telephone support and mobile responses as needed.
8. Collaborate with Region 1 CSBs, community partners/stakeholders, and parties involved in the consumer’s
treatment (including private providers, Department of Social Services (DSS)).
9. Suggest referrals to appropriate community services.
10. Completes other duties and projects as assigned.
