Four Oaks Family & Children Services in Cedar Rapids, IA is seeking an experienced professional to develop individualized care plans for youth in a residential setting. The role involves active collaboration with clients, families, and referral agencies to ensure goals are met and progress is documented.

The position requires a Bachelor’s in social work or a related field with experience in child welfare; a Master’s in Human Services is preferred.

Qualifications

• Bachelor's Degree in social work or a related field plus two years of child welfare experience; or Master’s in Human Services.
• Experience with Foster/Adoptive families is preferred.
• Ability to work independently and as part of a team; strong interpersonal skills.
• Effective verbal and written communication skills.

Responsibilities

• Developing a care plan for each child with goals and projected dates of accomplishment.
• Involving the client, referral agency and family in planning whenever possible.
• Developing a plan relating to parental involvement for the child.
• Documenting case assessments and care plan implementation.
• Providing for scheduled in-person conferences with each resident.
• Providing a supportive atmosphere with leadership and guidance to the client.
• Coordinating internal and external activities of the youth.
• Serving as a liaison with the referring agency.
• Managing day-to-day program operations and ensuring health and well-being.
• Overseeing living activities and monitoring behaviors daily.

What you will do?

• Developing a care plan for each child containing goals and objectives with projected dates of accomplishment
• Involving the clients, referral agency and family in planning whenever possible
• Developing a specific plan relating to the involvement of the child’s parents.
• Participating in the documentation of case assessment.
• Documenting the implementation of the care plan.
• Providing for scheduled in-person conferences with each resident.
• Providing a supportive atmosphere for the youth with leadership and guidance to the client.
• Coordinating internal and external activities of the youth.
• Serving as a liaison with the referring agency.
• Providing a mechanism for dealing with day-to-day program operations.
• Oversees and maintains general health and well-being of the child
• Overseeing the living activities of the child.
• Monitoring and recording behaviors on a daily basis.
