Overview

Critical Illness Recovery Hospital

Case Manager (PRN)

Requires a current licensure in a clinical discipline either as a Nurse or a Respiratory Therapist OR Social Work SW/MSW (potential license per state guidelines).

And

Previous experience in critical care setting required.

Previous discharge planning experience highly preferred.

SHIFT:

*Availability to work a minimum of 2 shifts per month on an "as needed" basis - shifts would be scheduled on weekdays, from approximately 8 a.m. to 5 p.m.*

Responsibilities

The Case Manager's responsibilities include:

  • Developing and implementing patient-specific, safe, and timely discharge plans.
  • Verifying utilization criteria reviews.
  • Building relationships and coordinating with payor sources to ensure proper reimbursement and promote cost-attentive care.
  • Ensuring compliance with facility-wide Utilization Management policies and procedures.
  • Coordinating UR compliance with Quality Management to meet licensure and accreditation requirements.
  • Managing fiscal responsibilities by maximizing reimbursement and reviewing insurance verification forms to minimize risks.
  • Facilitating multidisciplinary team meetings with physicians, nurses, respiratory therapists, and rehabilitation therapists.
Qualifications

Minimum requirements include:

  • Current licensure in a clinical discipline (Nurse RN/LPN/LVN, Respiratory Therapist, or Social Work license) per state guidelines.
  • Experience in inpatient hospital settings with critical or acute care patients (e.g., ICU, step-down, med-surg, vents).
  • Experience in medical case management and confidence in managing care plans for critically ill populations.

Preferred qualifications:

  • Experience in Care Management and Discharge Planning.
  • Knowledge of the insurance industry and government reimbursement processes.

Availability to work a minimum of 2 shifts per month on an "as needed" basis, scheduled on weekdays from approximately 8 a.m. to 5 p.m.
