Job summary

The Calvary Center in Phoenix seeks a Case Manager to coordinate and support recovery services, under Clinical Director guidance. The role focuses on care planning, discharge coordination, and collaborative care with a multidisciplinary team.

This PRN position offers professional growth through HealthStream online CEUs, robust continuing education, and a supportive leadership structure within UHS networks.

Qualifications

• Bachelors degree required; Master's degree preferred.
• Minimum of 2 years of experience in case management related work.
• Valid Fingerprint Clearance Card.
• Completion of BLS and First Aid Training within 30 days of employment.
• Completion of Verbal De-Escalation Training within 30 days of employment.

Responsibilities

• Coordinate and facilitate the effective delivery of quality services to Calvary patients.
• Maintain treatment plans, utilization review, and discharge planning.
• Lead psychoeducation groups and case management activities.

Case Manager Position Summary:

The Case Manager works under the guidance of the Clinical Director while mentoring case management staff. The Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality services to Calvary patients. The Case Manager is knowledgeable about and responsible for treatment plans, utilization review, discharge planning, psychoeducation groups, and case management.
